  43. <p>King Charles will resume public duties next week after being diagnosed with cancer, Buckingham Palace announced Friday. </p>
  44. <p>“His Majesty the King will shortly return to public duties following a period of treatment and recovery following his recent cancer diagnosis,” the palace said in a brief statement.</p>
  45. <p>However, it was not immediately clear whether the 75-year-old was still receiving treatment. </p>
  46. <p>To mark his public return, King Charles and Queen Camilla will visit a cancer treatment center on Tuesday, the palace said. </p>
  47. <p>King Charles will resume his public duties soon after his cancer diagnosis, Buckingham Palace said on Friday, releasing the above image. </p>
  48. <p>“This visit will be the first of a series of external engagements His Majesty will undertake in the coming weeks,” the palace said.  </p>
  49. <p>“In addition, at the request of the British Government, the King and Queen will host their Majesties the Emperor and Empress of Japan on a state visit in June.”</p>
  50. <p>A new photo was also released on Friday showing a smiling King Charles hugging his wife. </p>
  51. <p>“As the first anniversary of the Coronation approaches, Their Majesties remain deeply grateful for the many kindnesses and good wishes they have received from around the world throughout the joys and challenges of the past year,” the palace said. </p>

  53. <p>This comes after the monarch, who began outpatient treatment in February, was forced to put the brakes on much of his royal agenda to focus on his health.</p>
  54. <p>And although he responded &#8220;very well&#8221; to treatment, his health has reportedly deteriorated significantly in recent weeks, leaving him feeling &#8220;very poorly,&#8221; according to insiders.</p>
  55. <p>Charles was also reportedly &#8220;frustrated&#8221; with the current appearance of the monarchy after the family was plagued by several health problems in recent months.</p>
  56. <p>Just a month after revealing his cancer diagnosis to the world, his daughter-in-law Kate Middleton revealed that she too is in the midst of battling cancer.</p>

  58. <p>With two senior members of the firm undergoing treatment for their respective diagnoses, the royal family has been forced into a major reshuffle.</p>
  59. <p>The royal family&#039;s serious health problems emerged less than a year after the king&#039;s coronation last May. </p>
  60. <p>Additionally, the palace is regularly updating plans for Charles&#039; upcoming funeral, with multiple sources telling the Daily Beast that the situation is &#8220;not good.&#8221;</p>

  62. <p>“Of course he is determined to beat it and they are doing everything they can.  Everyone remains optimistic, but he is doing really badly.  More than they admit,” a source described as an old friend of the royal family told the outlet.</p>
  63. <p>News of Charles&#039; diagnosis came to light in February after he underwent surgery for an enlarged prostate.  There, doctors discovered “another point of concern” that required treatment.</p>
  64. <p>Buckingham Palace later clarified that the cancer was not prostate cancer.</p>

  66. <p>But behind the scenes, King Charles&#039;s staff regularly review copies of a several hundred-page document called &#8220;Operation Menai Bridge,&#8221; which outlines his royal funeral plans.</p>
  67. <p>The Death Plan is named after the suspension bridge that connects the island of Anglesey to mainland Wales.</p>
  68. <p>“The plans have been dusted off and are being actively kept up to date.  It&#039;s no more than you&#039;d expect considering the king was diagnosed with cancer.  But their circulation has certainly sharpened minds,” a former staffer with ties to the incumbent courtiers told the Daily Beast.</p>

  90. <p>Private Medicaid health plans lost millions of members in the past year as pandemic protections that prohibited states from dropping anyone from the government program expired.</p>
  91. <p>But despite Medicaid’s unwinding, as it’s known, at least two of the five largest publicly traded companies selling plans have continued to increase revenue from the program, according to their latest earnings reports.</p>
  92. <p>“It’s a very interesting paradox,” said Andy Schneider, a research professor at Georgetown University’s McCourt School of Public Policy, of plans’ Medicaid revenue increasing despite enrollment drops.</p>
  93. <p>Medicaid, the state-federal health program for low-income and disabled people, is administered by states. But most people enrolled in the program get their health care through insurers contracted by states, including UnitedHealthcare, Centene, and Molina.</p>
  94. <p>The companies persuaded states to pay them more money per Medicaid enrollee under the assumption that younger and healthier people were dropping out — presumably for Obamacare coverage or employer-based health insurance, or because they didn’t see the need to get coverage — leaving behind an older and sicker population to cover, their executives have told investors.</p>
  95. <p>Several of the companies reported that states have made midyear and retrospective changes in their payments to plans to account for the worsening health status of members.</p>
  96. <p>In an earnings call with analysts on April 25, Molina Healthcare CEO Joe Zubretsky said 19 states increased their payment rates this year to adjust for sicker Medicaid enrollees. “States have been very responsive,” Zubretsky said. “We couldn’t be more pleased with the way our state customers have responded to having rates be commensurate with normal cost trends and trends that have been influenced by the acuity shift.”</p>
  97. <p>Health plans have faced much uncertainty during the Medicaid unwinding, as states began reassessing enrollees’ eligibility and dropping those deemed no longer qualified or who lost coverage because of procedural errors. Before the unwinding, plans said they expected the overall risk profile of their members to go up because those remaining in the program would be sicker.</p>
  98. <p>UnitedHealthcare, Centene, and Molina had Medicaid revenue increases ranging from 3% to 18% in 2023, according to KFF. The two other large Medicaid insurers, Elevance and CVS Health, do not break out Medicaid-specific revenue.</p>
  99. <p>The Medicaid enrollment of the five companies <a rel="nofollow" href="https://www.kff.org/medicaid/issue-brief/experience-of-the-five-largest-publicly-traded-companies-operating-medicaid-managed-care-plans-during-unwinding/">collectively declined</a> by about 10% from the end of March 2023 through the end of December 2023, from 44.2 million people to 39.9 million, KFF data shows.</p>
  100. <p>In the first quarter of 2024, UnitedHealth’s Medicaid revenue <a rel="nofollow" href="https://www.unitedhealthgroup.com/content/dam/UHG/PDF/investors/2024/UNH-Q1-2024-Release.pdf">rose to $20.5 billion</a>, up from $18.8 billion in the same quarter of 2023.</p>
  101. <p>Molina on April 24 reported nearly $7.5 billion in Medicaid revenue in the first quarter of 2024, up from $6.3 billion in the same quarter a year earlier.</p>
  102. <p>On April 26, Centene reported that its Medicaid enrollment fell 18.5% to 13.3 million in the first quarter of 2024 compared with the same period a year ago. The company’s Medicaid revenue dipped 3% to $22.2 billion.</p>
  103. <p>Unlike UnitedHealthcare, whose Medicaid enrollment fell to 7.7 million in March 2024 from 8.4 million a year prior, Molina’s Medicaid enrollment rose in the first quarter of 2024 to 5.1 million from 4.8 million in March 2023. Molina’s enrollment jump last year was partly a result of its having bought a Medicaid plan in Wisconsin and gained a new Medicaid contract in Iowa, the company said in its earnings news release.</p>
  104. <p>Molina <a rel="nofollow" href="https://investors.molinahealthcare.com/news-releases/news-release-details/molina-healthcare-reports-first-quarter-2024-financial-results">added 1 million members</a> because states were prohibited from terminating Medicaid coverage during the pandemic. The company has lost 550,000 of those people during the unwinding and expects to lose an additional 50,000 by June.</p>
  105. <p>About 90% of Molina Medicaid members have gone through the redetermination process, Zubretsky said.</p>
  106. <p>The corporate giants also offset the enrollment losses by getting more Medicaid money from states, which they use to pass on higher payments to certain facilities or providers, Schneider said. By holding the money temporarily, the companies can count these “directed payments” as revenue.</p>
  107. <p>Medicaid health plans were big winners during the pandemic after the federal government prohibited states from dropping people from the program, leading to a surge in enrollment to about 93 million Americans.</p>
  108. <p>States made efforts to limit health plans’ profits by clawing back some payments above certain thresholds, said Elizabeth Hinton, an associate director at KFF.</p>
  109. <p>But once the prohibition on dropping Medicaid enrollees was lifted last spring, the plans faced uncertainty. It was unclear how many people would lose coverage or when it would happen. Since the unwinding began, more than 20 million people have been dropped from the rolls.</p>
  110. <p>Medicaid enrollees’ health care costs were lower during the pandemic, and some states decided to exclude pandemic-era cost data as they considered how to set payment rates for 2024. That provided yet another win for the Medicaid health plans.</p>
  111. <p>Most states are expected to complete their Medicaid unwinding processes this year.</p>

  135. <p>Donald Trump returned to court this morning for the final part of his New York hush money trial, where the jury heard more testimony from a former tabloid mogul who detailed the &#8220;catch-and-kill&#8221; plan at the heart of the case.</p>
  136. <p>Former National Enquirer editor David Pecker testified about his agreement with his &#8220;mentor&#8221; Mr. Trump and ex-lawyer Michael Cohen to suppress stories about the defendant&#039;s alleged affairs. </p>
  137. <p>After cross-examination of Mr. Pecker by defense attorney Emil Bove, the witness doubled down during further questioning by the prosecution on his testimony that he had bought and silenced the story of Playboy model Karen McDougal&#039;s affair with Mr. Trump to influence the 2016 election had brought.</p>
  138. <p>Judge Juan Merchan has yet to decide whether Mr. Trump should be held in contempt and fined $10,000 for allegedly violating a court banning order by posting about witnesses and jurors on Truth Social.</p>
  139. <p>In other legal news, Supreme Court justices heard oral arguments on Mr. Trump&#039;s &#8220;presidential immunity&#8221; defense against prosecution in federal cases, and the former president&#039;s recent efforts to overturn the E. Jean Carroll defamation judgment against him were dismissed.</p>

  167. <p>There is no evidence that President Joe Biden in any way orchestrated any of the civil or criminal charges against Mr. Trump.  The federal cases were handed over by the Justice Department to special counsel Jack Smith to guard against possible government interference.  The state and local cases are outside the jurisdiction of the federal government.  All four criminal cases were presented to grand juries of U.S. citizens, who recommended indictments.</p>

  258. <p>Nine Virginia Tech Carilion School of Medicine (VTCSOM) faculty, staff and students have been named 2024 Dean&#039;s Diversity Champions.  This is the third year the school has recognized individuals for significant achievements in developing a more diverse and inclusive community.</p>
  259. <p>“I commend these individuals for prioritizing diversity at VTCSOM,” said Dean Lee Learman.  “As one of our core values, diversity and inclusion affirm our shared humanity and enhance the quality of our learning environment.”</p>
  260. <p>It is well documented that diversity in medicine also leads to better patient outcomes.</p>
  261. <p>“It is important that health systems do everything we can to implement policies and processes that improve the lives of our patients.  Diversity is part of that answer,” said Azziza Bankole, the medical school’s chief diversity officer.</p>
  262. <p>Bankole added: “Recognizing members of the VTCSOM community who have demonstrated their commitment to diversity highlights what we value as a school and is not only an example for others to begin such work, but also a blessing for many to continue their efforts.”</p>
  263. <p>Students, faculty and staff nominated the recipients, who were then selected by the school&#039;s inclusion and diversity team.  Some of the activities in which the honorees participated included leading an after-school health program in an underserved Roanoke neighborhood, hosting a community-wide gathering for Hispanic Heritage Month, providing literacy instruction to local immigrant and refugee communities, and the volunteered for the school&#039;s Hispanic Heritage Month MedDOCS program.</p>

  283. <p id="IM4RPU">Collar City Mushrooms is located in a small building on the post-industrial waterfront in Troy, New York.  Outside, baking in the winter sun between a shed and a yellow Volkswagen bus, lies a waist-high pile of what looks like dozens of giant Frosted Mini-Wheats, each about the size of a cinder block.  The weathered caps of oyster mushrooms sprout defiantly in various parts of the pile.</p>
  284. <p id="UmCBzS">The lumpy blocks are spent substrate, the living material left over after mushroom cultivation.  Composed of sawdust and soy pellets laced with mycelium – the threadlike aspect of the fungus from which mushrooms sprout – spent substrate is a unique type of waste.  It is also one with many uses;  It can be used as compost, to decontaminate soil, as biofuel, and simply to grow more mushrooms.  And while each of these uses could represent revenue potential for mushroom farms, the growing amounts of substrate consumed also pose a growing logistical challenge.</p>
  285. <p>“If you do, great, but consider the waste stream you produce and how you remove it from your property.”</p>
  286. <p id="P1Dxfc">“Right now there are people who are picking it up almost as a favor for us, because what else are we going to do with it?” Collar City co-founder Avery Stempel said as we looked at the stack.  Stempel currently takes most of the material to a nearby compost facility, but local farms, gardeners and florists also take some.  This also applies to individuals, whether for compost in their garden or simply for growing mushrooms at home.  “People will come and buy a bucket for five dollars,” Stempel said.</p>
  287. <p id="4u8ECm">Before mushroom cultivation begins, the substrate is carefully mixed and sterilized to maximize efficiency and prevent competition for the mushroom.  The sawdust and soy hulls, protected in breathable plastic bags, are inoculated with a strain of edible mushrooms and then stacked on racks in air-conditioned rooms.  The bags are cut open when the mycelium is ready and the first mushrooms sprout out.  To make the best use of space, many farms discard the blocks after a single flush, but each block can produce mushrooms for several rounds.  In this sense, the substrate is not really “used”.</p>

  293. <p id="upn4Wv">Collar City is a relatively small operation that produces up to 1,000 pounds of mushrooms per week.  An hour south, in Hillsdale, New York, Tivoli Mushrooms produces about 20,000 pounds per week and is currently using only half the capacity of its new 15,000-square-foot facility.  Shortly after moving in, co-founder Devon Gilroy contacted a neighboring organic farm and offered the used substrate as compost for free if they would simply take it from him.  It wasn&#039;t a hard sell.  “They arrived about two weeks later with a tractor and a large truck to load it,” he said.  “They insisted on paying us for the substrate, which really helped.”</p>
  294. <h3 id="Vqcskw">More mushrooms, more problems</h3>
  295. <p id="aAYqB1">From a sales perspective, the greatest value of specialty mushroom substrate currently lies in compost, which can sell for around $150 per cubic meter.  It has a low pH, making it suitable for soils with low acidity and a carbon to nitrogen ratio of around 40 to 1, which is almost ideal for building healthy soils.  Spent substrate is also a useful addition to vermicompost &#8211; worms like to eat mycelium, decompose wood residues and support biodiversity in the soil.  It is also an excellent addition for structure and water retention.</p>
  296. <p id="lYweny">However, that doesn&#039;t mean every mushroom farm has an easy time finding a second life for their spent substrate, and the dilemma over using the material grows with the size of the specialty mushroom industry.</p>
  297. <p id="5XpQ54">“If you do, great, but consider the waste stream you produce and how you remove it from your property.” That&#039;s the advice Amanda Janney, founder of KM Mushrooms in California, gives new farmers.  Janney&#039;s farm is as humble as it gets, operating out of her home in Santa Rosa.  As the farm&#039;s production increased rapidly, from 20 pounds of mushrooms per week to about 300, the leftover material quickly became a logistical problem that needed to be solved.</p>
  298. <p id="AtWvPY">“In the beginning, when we were playing at very low volumes, it wasn&#039;t a big consideration;  All it took was distributing bags of spent substrate through Craigslist and Facebook Marketplace,” Janney said.  “Then production increased much faster than I had planned, which is a great thing, but a big part of it was connecting with interested farmers.” [taking substrate] and create a workflow to quickly remove it from the property.”</p>
  299. <p id="fvg19d">In 2022, global mushroom sales are expected to more than double, rising to over $110 billion by 2030, driven in large part by consumer interest in meat alternatives.  The nutraceutical market for medicinal mushrooms – such as reishi, lion&#039;s mane and cordyceps – could follow a similar trajectory, with one forecast suggesting the market could triple to reach $62 billion by 2032.</p>
  300. <p id="0i2Uw5">The vast majority – 95 percent – ​​of mushroom production in the U.S. is Agaricus: the common cremini, button, or portobello (all the same species).  Any other variety, be it shiitake or oyster, falls into the specialty mushroom category.</p>
  301. <p id="tTEwE8">In the United States, Agaricus mushrooms are produced in large quantities by established farms, often generations old, mostly located in Kennett Square, Pennsylvania.  When grown in a combination of manure and straw, they produce a special type of spent substrate that is also used as compost, although it is a very different material than special mushroom substrate and has less application.  Growing mushrooms inexpensively is very labor and space intensive, and growing them on a small scale makes little economic sense.</p>

  303. <p id="LfdzVf">In comparison, it is relatively easy to grow enough oyster mushrooms to sell on the market for more than $10 per pound, compared to about $5 per pound for button mushrooms.  It&#039;s also easy to buy a few ready-made kits for home growing.  For these and other reasons, most small and emerging farms will likely grow specialty mushrooms.  Sales of specialty mushrooms increased 32 percent between 2021 and 2022 alone, meaning a corresponding increase in substrate consumed and more questions about what to do with it.</p>
  304. <p id="uRcc2f">At the extreme end of the substrate volume are the emerging mycelial materials companies such as MycoWorks and Ecovative. [Disclosure: The author worked for Ecovative in 2022 and 2023.] Ecovative is based on oyster mushroom mycelium and sends most of its substrate as compost.  Among the possible uses the company is exploring is selling some of its massive production of leftover substrate to farms in Pennsylvania for a second act of mushroom production.</p>
  305. <p id="t1rEJU">But that market has a limit: &#8220;To be honest, you couldn&#039;t possibly eat enough oyster mushrooms in the U.S. to use up all the substrate we&#039;ll make if we reach our goal,&#8221; said Eben Bayer, CEO of Ecovative.  &#8220;On a spreadsheet it&#039;s a huge opportunity, but on the operational side it&#039;s &#039;Get this stuff out of here now.&#039;&#8221;</p>
  306. <h4 id="dl8kxY"><strong>A community solution for substrate waste is emerging in Texas</strong></h4>
  307. <p id="atHx7d">Rather than relying solely on the market, the question of what to do with the substrate is largely answered by the local communities of specialty mushroom farms.  In Austin, for example, the Central Texas Mycological Society (CTMS) has organized a network of about two dozen locations for free pickup of used substrates.  They report that around 9,000 people have signed up since the program began three years ago, with the increase increasing during the pandemic as interest in home-grown mushrooms increased dramatically.</p>
  308. <p id="rO2B5K">“With the spent mushroom substrate, we saw an opportunity to keep people connected to each other,” said Angel Schatz, one of the main organizers of the CTMS, whose front yard was the original drop-off point for the program.  However, what people do with the material is their own business.  “I know a lot of people grow the mushrooms and get a second harvest from the bags, but we don&#039;t want to take the wind out of the sails of commercial farms in any way, so we&#039;ll start by teaching people composting methods.  ”</p>

  314. <p id="i5Gf5a">Until recently, a significant portion of the substrate used for the CTMS pickup program came from Smallhold, which quickly became a leading specialty mushroom grower over the past five years before filing for bankruptcy in early February.  With offices in Los Angeles, Austin, and New York, the company&#039;s goal was to grow specialty mushrooms near major cities.  Each of its three large operations produced approximately 80 to 100 cubic meters of spent substrate per week, and the company employed a team dedicated to finding productive uses for the material.</p>
  315. <p id="1LrLL3">“At the end of the day, this is a valuable material,” said Travis Breihan, who was responsible for researching uses for spent substrate as the company’s impact manager.  “But it&#039;s a new material on the scene, and it&#039;s not like there&#039;s an established industry where people are flushing blocks for the second time or using it as a garden improvement or even a major agricultural amendment.  So I think it’s still early days for adoption, but all signs point very strongly to this being a big focus area for the mushroom industry as a whole.”</p>
  316. <p id="uvHK63">CTMS does not worry about losing the smallhold substrate.  “We are still working with another farm that produces about 1,900 spent substrate blocks weekly and these will probably grow now because Smallhold won’t be here.</p>
  317. <p id="eIMY5m">In addition to giving away blocks, CTMS now also works with local farms, food producers and environmental remediation projects that use the material to clean up contaminated sites, such as the Circle Acres Nature Preserve on the edge of Austin.</p>
  318. <p id="Lag0To">In fact, given the limitations that scale imposes on the transportation and productive use of spent substrate, the future market for the material is likely to be dominated by smaller operations.  Special mushrooms are suitable for this dynamic.  They are not well suited to long-distance transport and can run on waste streams – such as sawdust – from neighboring industrial plants.  It can take many different shapes and, above all, sizes.  As the specialty mushroom industry grows, spent substrate could find a market for secondary mushroom production or for land and waterway construction and rehabilitation.  The material&#039;s potential can best be realized by linking mushroom production to other food and soil-based initiatives and supporting a more circular, regional economy.</p>
  319. <p id="2xIvX8">“The least we can do is make sure the cycle is complete and put it back in the ground instead of sending it to a landfill,” Schatz said.</p>

  380. <p>Lord Cameron declined to comment on his conversations with the former president at Mar-a-Lago, but described him on Wednesday as &#8220;certainly an interesting guy.&#8221;  In his 2019 memoir, the former prime minister said Mr Trump was “xenophobic” and “misogynist”.</p>
  381. <p>At the dinner, Lord Cameron tried to persuade Mr Trump over a steak dinner that he should support Ukraine&#039;s war effort by arguing that Vladimir Putin would continue to invade European countries if he was not stopped by the West, reported the Washington Post.</p>
  382. <p>A readout of the meeting from Mr. Trump&#039;s campaign simply referred to the need to &#8220;stop the killing&#8221; during the war, apparently referencing his preferred policy of a negotiated ceasefire.</p>
  383. <p>It was also reported on Friday that British diplomats had private dinner with the former president, attended his Super Tuesday victory party and held meetings with Liz Truss and Nigel Farage &#8211; both of whom have a better relationship with Mr Trump than with the British government.</p>

  397. <p>Meta Platforms shares plummeted yesterday, causing the assets of owner and CEO Mark Zuckerberg to plummet.</p>
  398. <p class="Normal">Closing the session on April 25, Meta Platforms shares decreased 11%. This caused CEO Mark Zuckerberg’s assets to lose $18 billion. According to Bloomberg Billionaires Index, this is the 11th largest daily decrease they have ever recorded among the world’s billionaires.</p>
  399. <p class="Normal">Zuckerberg’s assets mainly come from Meta shares. He currently owns 157 billion USD. This causes the Meta boss to step back to the position of the 4th richest person on the planet, making room for Tesla CEO Elon Musk.</p>
  400. <p class="Normal">Musk yesterday had an additional 5.8 billion USD thanks to Tesla shares going up. He currently owns 184 billion USD. Earlier this month, Zuckerberg surpassed Musk, richer than Musk for the first time since 2020. Since the beginning of the year, he has an additional 29 billion USD.</p>
  401. <p class="Normal">Meta shares plunged yesterday after the company said second-quarter revenue may be lower than analysts’ forecasts. The company also raised its investment forecast in new AI products and artificial intelligence support infrastructure this year, to 96-99 billion USD. Spending levels may continue to increase next year.</p>
  402. <p class="Normal">In recent months, Zuckerberg has said artificial intelligence is a priority for the company. They turned their focus to this technology after OpenAI launched ChatGPT chatbot in 2022, creating a global artificial intelligence fever.</p>
  403. <p class="Normal">However, this information makes investors worried. Meta’s bet on artificial intelligence may take years to pay off. However, the actual decrease yesterday was lower than the 15% level in the April 24 after-hours trading session.</p>
  404. <p class="Normal">Since the beginning of the year, this stock has increased 25%. Over the past month, this code has continuously approached its historical peak, partly thanks to investors’ excitement about AI technology.
