  93.                <p>This article explores the impact of the widespread lockdowns during the COVID-19 pandemic on daily routines and activity levels, with a specific focus on their potential to exacerbate the risk of type 2 diabetes (T2D), a significant concern
  94.                    for U.S. adults due to its strong association with weight gain. The study involved surveys conducted among affiliates from TOURO college campuses to assess how pandemic-related changes affected dietary habits, daily activity levels,
  95.                    and weight among participants. To quantify changes in T2D risk, researchers calculated a diabetes risk score by comparing pre-pandemic and pandemic data. The findings were striking: 65.9% of respondents reported decreased activity
  96.                    levels, 46.3% experienced weight gain, and an alarming 64.2% exhibited elevated risk scores, indicating an increased vulnerability to T2D. These results underscore the potential health consequences of the pandemic-induced disruptions
  97.                    to daily life and emphasize the need for proactive measures in public health planning to address these issues.</p>

  100.                <h1><a href="https://www.acofpca.org/page/JOFPCAFall2023AddressingGlobalHealthCompetencies" target="_blank">Addressing Global Health Competencies Through a Virtual Elective During the COVID-19 Pandemic</a></h1>

  102.                <p>Due to the COVID-19 pandemic, the Global Health Department at Touro University California College of Osteopathic Medicine adapted its curriculum to deliver it virtually for the first time. We enrolled 25 first- and second-year medical
  103.                    students in a Global Health elective of 12 sessions in the Spring of 2021. Students received pre-recorded video lectures and study materials for each session, followed by live interactions with international preceptors. We analyzed
  104.                    the effectiveness of virtual sessions and overall learning outcomes by using 1) a self-proctored multiple-choice quiz administered before and after each session for eight sessions and 2) true-false surveys at the end of each session
  105.                    for ten sessions. Students’ mean score on pre-session quizzes was 56.33% (standard deviation of 1.72%), with individual quiz scores ranging between 42.22% and 79.53%. However, after live interactions via Zoom, students performed significantly
  106.                    better, with a mean score of 89.55% (p &lt;0.001, standard deviation of 1.78%) and individual quiz scores between 69.08% and 100%. When surveyed about their attitudes towards provided topics and study materials and confidence in discussing
  107.                    the topics and their future implications, students’ response was positive, between 95.8% and 99.3%, with an overall survey response rate of 96.7%. We believe that virtual learning provided an effective platform to address global health
  108.                    competencies during the COVID-19 pandemic. It can, therefore, be an effective model for maintaining the engagement of health science students in global health education.</p>

  113.                <p>The opioid epidemic has been of significant concern in American society as it continues to worsen. Overdose deaths remain a leading cause of injury-related death in the United States. The majority of overdose deaths involve opioids. Women
  114.                    are often exposed to opioids for the first time postpartum. Opioid administration postpartum has become common practice, not to discredit postpartum pain, which is often excruciating. Pomona Valley Hospital Medical Center led a quality
  115.                    improvement (QI) initiative aimed at reducing the burden of opioid use in pregnant patients, especially postpartum. Over time, Pomona Valley Hospital introduced several indirect and direct interventions to provide our patients with
  116.                    the comfort they desired and deserved. The primary outcome and impact of our initiative and implementation saw a significant reduction in the overall necessity of opioid administration, along with improved outcomes for our patients
  117.                    concerning comfort, care, and alleviation of symptoms postpartum. Our intervention shows that narcotic use is not necessary for postpartum care, and more conservative measures can often control symptoms. There will be breakthrough
  118.                    cases requiring opioid administration, but for the vast majority of pregnancies, pain can often be controlled by non-narcotic means. With the worsening nature of the opioid epidemic, we must lessen the burden narcotics have placed
  119.                    on our society. As a mother’s first exposure to opioids occurs commonly during and after pregnancy, it is essential to break the cycle and work towards reducing opioid use and the possibility of addiction through early intervention
  120.                    by encouraging conservative means for pain management.</p>

  125.                <p>Objectives: We aim to elucidate the effects of cannabis use during pregnancy on maternal and neonatal outcomes.</p>
  126.                <p>Methods: Electronic medical records were reviewed to identify subjects who had clinic visits to our county hospital between November 2018 and November 2021, had any ICD code beginning with Z3A (indicating gestation), and had a Urine Drug
  127.                    Screen (UDS) performed. We identified UDS results positive for cannabis and time-based controls for cannabis negative results. We performed a retrospective cohort study with n=370 and collected data on maternal and neonatal outcomes.
  128.                    Statistical analyses were performed using standard data analysis techniques, including independent t-test and crosstabulation chi-square analyses, to assess differences between marijuana-positive groups vs. marijuana-hostile groups.</p>
  129.                <p>Results: We found significant associations between cannabis use during pregnancy and increased prevalence of comorbid mood disorders and psychotic disorders. No statistically significant differences in anxiety disorders were found between
  130.                    the two groups. Additionally, we found a significant association between cannabis is used during pregnancy and increased failures in newborn hearing screenings.</p>
  131.                <p>Conclusions: Our study suggests that cannabis use during pregnancy may be associated with comorbid mood disorders, psychotic disorders, and neonatal hearing deficits. These findings indicate that the safety profile of cannabis use during
  132.                    gestation may be discordant with the widespread understanding of this substance.</p>

  149.                <p>This systematic review examines the potential therapeutic applications of Cold Water Immersion (CWI) for mental health. CWI activates the sympathetic nervous system, releasing stress hormones and endorphins, which can improve mental well-being.
  150.                    Physiological benefits include anti-inflammatory effects, improved circulation, and enhanced immune function. CWI shows promise as an adjunct treatment for depression, anxiety, pain-related disorders, and stress, but safety considerations
  151.                    are vital, including the risk of hypothermia, cardiovascular and respiratory concerns, and the cold shock response. It should complement, not replace, traditional treatments and is best suited for specific patient populations. In conclusion,

  170.                <p>Herpes zoster, known as shingles, typically presents with a well‑defined dermatomal rash, but atypical manifestations can post diagnostic challenges. We present the case of a 55‑year‑old African American woman with a history of childhood
  171.                    varicella‑zoster virus (VZV) vaccination who presented with a painful perianal rash initially mistaken for an allergic reaction. Despite previous vaccination, she developed a series of asymmetric ulcerative lesions with associated
  172.                    itching and burning, extending into the perianal region. Visual inspection revealed vesicular lesions on the right medial buttock, suggestive of shingles.</p>

  177.                <p>Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) is a rare and potentially life-threatening adverse drug reaction. We present a case of a 14-year-old girl with overlapping DRESS syndrome and Toxic Shock Syndrome (TSS) following
  178.                    Trimethoprim/Sulfamethoxazole treatment. The patient developed a generalized pruritic maculopapular rash, lymphadenopathy, fever, and facial swelling. Laboratory results showed leukocytosis, elevated liver enzymes, and C-reactive protein
  179.                    (CRP). Despite initial treatment with methylprednisolone, her condition worsened, necessitating a transfer to a higher level of care. She exhibited fever, hypotension, tachycardia, tachypnea, strawberry tongue, and perioral impetigo,
  180.                    consistent with TSS. Antibiotic therapy was initiated, but eosinophilia and an abdominal desquamative rash emerged, indicative of DRESS syndrome. Treatment involved discontinuing antibiotics, administering intravenous solumedrol for
  181.                    possible DRESS, and restarting antibiotics due to persistent TSS features. The patient’s symptoms improved, and she was discharged on tapering steroids and antibiotics. This case highlights the coexistence of DRESS and TSS in a pediatric
  182.                    patient and emphasizes the importance of promptly discontinuing the offending agent and appropriate management with steroids and antibiotics.</p>

  188.                    abdominal pain. Due to the patient’s chronic kidney disease, two non-contrast CT scans initially ordered showed unremarkable results. The decision to order a third CT scan with contrast finally demonstrated a thrombus in the superior
  189.                    mesenteric artery with potential bowel ischemia. The patient’s condition deteriorated rapidly during the discovery of mesenteric artery thrombosis and extensive gangrenous bowel during surgery. Due to the irreversibility of the bowel

  435.    <p><strong>Rancho Cucamonga, CA —</strong> The American College of Osteopathic Family Physicians of California (ACOFP-CA) Board is pleased to announce the 2023 Resident Writing Award winner. This accolade is awarded to a resident physician ACOFPCA member who presents an intriguing family medicine case in which they have been involved. This year's honor goes to Samarth Bhatnagar, DO, an alumnus of Touro University California College of Osteopathic Medicine.</p>
  436.    <p>Dr. Bhatnagar's winning submission, "Gout: A Case Study," is a testament to his outstanding clinical acumen and educational contribution to family medicine. His paper thoroughly discusses the management of a particularly challenging case of gout with atypical presentations and complex comorbidities, including Chronic Kidney Disease (CKD3b) and Congestive Heart Failure (CHF).</p>
  437.    <p>In recognition of his case study, which highlighted his direct involvement and management of a nuanced family medicine case, Dr. Bhatnagar has been awarded a $500 prize. His approach, which resulted in a significant clinical improvement through the innovative use of Allopurinol and Pegloticase, demonstrates the critical impact of dedicated, patient-centered care.</p>
  438.    <p>The selection process, carried out by a blind panel of judges, evaluated submissions based on their originality, substance, and the logical flow of medical arguments, amongst other rigorous criteria.</p>
