94.

 96.

 97.
Ob/GYN Billing Denial Codes

 98.
Compared to more commonly processed claims in pediatrics, rheumatology and other branches of medicine, OB/GYN practitioners find themselves facing an alarming percentage of insurance denials often nearly double that of other practitioners. In this post, we'll discuss which CDT codes are most often denied according to a report from RemitDATA, what reason is most commonly given for them and how to avoid having claims with those codes denied.

 99.
It should be noted that implementing medical records systems that help reduce errors or make it easier to view claims with a problematic code help reduce denials. Recent articles on electronic health records systems by both Information Week and Med City News pointed to reduced errors, ease of searching and discovering patterns in denials and increased patient interaction.

 100.
99000 - Specimen Handling Office/Lab

 101.
Whether the lab is across the street or across the country, code 99000 refers to expenses that are incurred in transporting a specimen to a lab, such as time, fuel, mileage, postage or using a courier service, this would be an appropriate code. If it doesn't incur significant costs, you'll see denial code 97: The benefit for this service is included in the payment/allowance for another service/procedure that has already been adjudicated.

 102.
99213 - Outpatient Doctor Visit, Level 3

 103.
You have a routine office visit with your patient and use a routine code, 99213, to bill it. Unfortunately, too many practitioners use this code as a one-size-fits-all generic code, causing it to be the most commonly denied code with a reason code of 18, duplicate claim or service. By definition, a level three visit has to be in depth enough to make medical decisions of low complexity. If you're doing more than that, you should brush up on other codes for common issues.

 104.
81002 - Urinalysis Non-Automated W/O Scope

 105.
Code 81002 tends to be a catch-all for basic urinalysis procedures whether for ketones, biliruben or glucose. Whatever the test was for, adding a modifier code often helps you avoid denial code 16: Claim lacks information or has errors.

 106.
36415 - Routine Blood Capture

 107.
You would think that billing a procedure that involves sticking a needle in someone's arm and collecting the blood from it would be pretty commonplace, right? Yet routine blood capture is the third-highest denied code, often due to not being paid separately, reason code 234.

 108.
99214 - Outpatient Doctor Visit, Level 4

 109.
By comparison, 99214 requires decision making that is moderately complex and often requires testing to decide the best possible treatment for your patient. If you had to talk to your patient about their new medication in depth because they don't understand the difference, it's a level 3 visit. If you're prescribing a new medication based on an in-depth interview, testing and above-average research, it's level 4. This is why reason code 96, non-covered charge(s), is the most common denial.

 110.
In addition to the above suggestions, putting appropriate coding and documentation procedures into place in your practice also help lower denials significantly. A recent University of Mississippi Medical Center slide show presents a number of excellent suggestions for lowering denials through better office procedures, including suggestions on the upcoming changeover from ICD-9 to ICD-10.

 111.
At RevenueXL, our commitment is to help you provide the best possible care to your patients through electronic medical records designed specifically for OB/GYN practitioners, strategies to avoid insurance denials and professional outsourced medical billing, keeping you taking care of people instead of chasing insurance companies.

Yet routine blood capture is the third-highest denied code, often due to not being paid separately, reason code 234.

 127.
99214 - Outpatient Doctor Visit, Level 4

 128.

 129.

 192.

 193.
How Value-Based Care is Replacing Fee-for-Service as the Future of Healthcare Reimbursement

 194.
In the ever-evolving healthcare landscape, two distinct approaches have emerged as front runners: Fee-for-Service and Value-Based Care. These contrasting models shape how healthcare is delivered and have far-reaching implications for patients, providers, and the healthcare system. In this blog post, we'll dive into the fundamental differences between Fee-for-Service and Value-Based Care, exploring their strengths, weaknesses, and impact on the quality and cost of healthcare. Join us on this journey to better understand these pivotal healthcare paradigms.

 195.
 196.
What is Fee for Service in Healthcare?

 197.
Fee-for-service in healthcare is a payment model in which medical providers receive compensation for each service rendered to a patient. This arrangement typically involves patients paying out-of-pocket or through insurance for each individual medical procedure, consultation, or treatment received rather than a bundled or capitated payment structure. This means the more services a physician provides, the more they get paid, regardless of whether those services lead to better health outcomes for the patient.

 198.
Example: Imagine a patient visiting a doctor with flu-like symptoms.

 199.

 200.
The doctor charges a fee for the office visit.

 201.
If a blood test is recommended, there's a separate fee.

 202.
Suppose an X-ray is deemed necessary; yet another fee is billed.

 203.
Prescription medications or further referrals to specialists generate additional costs.

 204.

 205.
The bill reflects the sum of all these individual services, even if the patient's condition didn't improve or some tests were unnecessary. This model can lead to overuse of services and unnecessary treatments, which can drive up healthcare costs.

 206.
Many healthcare systems and insurance companies are moving away from fee-for-service models and toward value-based care, which focuses on providing high-quality, cost-effective care rather than simply providing more services. This shift incentivizes healthcare providers to focus on their patients' overall health and well-being rather than simply providing more services.

 234.
What are Current Procedural Terminology Codes and CPT Modifiers?

 235.

 236.

 237.
 238.

 239.

 240.
What is a CPT Code?

 241.

 242.

 243.

 244.
The Current Procedural Terminology (CPT®) code is a standardized medical code set designed to provide doctors, healthcare professionals, and administrative entities with a unified language, improve reporting accuracy, streamline processes, and increase efficiency. It is used for documenting and reporting medical, surgical, radiological, laboratory, anesthesiological, genomic sequencing, evaluation, and management (E/M) services.

 245.
CPT codes serve multiple roles:

 246.

 247.
Clinical Reporting: They help healthcare providers accurately report the various procedures, services, and interventions they administer to patients.

 248.
Administrative and Management: They are instrumental in administrative tasks, such as medical claims processing and creating guidelines for medical care review.

 249.
Electronic Medical Billing: They are used alongside ICD-9-CM or ICD-10-CM diagnostic codes during the electronic medical billing process to communicate information to entities like physicians, health insurance companies, and accreditation bodies.

 250.

 251.
Unique features of CPT codes:

 252.

 253.
Structure: Each CPT code consists of a five-digit numerical identifier that corresponds to a specific medical procedure or service.

 254.
Annual Updates: Managed by the American Medical Association (AMA) with input from professional specialty societies, including those focusing on pain management, the CPT manual undergoes regular annual updates to reflect the latest in medical practice and innovations.

 255.
Transparency and Rigor: The CPT® Editorial Panel, established over 50 years ago and convened by the AMA, oversees the CPT code set's development and management. This panel ensures a transparent, rigorous process that yields clinically valid codes, updating them as needed to mirror contemporary clinical practices and medical innovations.

 256.

 257.
Providers are tasked with the responsibility of understanding, documenting, and reporting CPT codes accurately for the services they deliver, adhering to the extensive coding requirements outlined in the CPT manual.

 258.

 259.

 265.
What is a CPT Code?

 266.

 267.

 268.

 269.

 270.
CPT codes serve multiple roles:

 271.

 272.
Clinical Reporting: They help healthcare providers accurately report the various procedures, services, and interventions they administer to patients.

 273.
Administrative and Management: They are instrumental in administrative tasks, such as medical claims processing and creating guidelines for medical care review.

 274.
Electronic Medical Billing: They are used alongside ICD-9-CM or ICD-10-CM diagnostic codes during the electronic medical billing process to communicate information to entities like physicians, health insurance companies, and accreditation bodies.

 275.

 276.
Unique features of CPT codes:

 277.

 278.
Structure: Each CPT code consists of a five-digit numerical identifier that corresponds to a specific medical procedure or service.

 279.
Annual Updates: Managed by the American Medical Association (AMA) with input from professional specialty societies, including those focusing on pain management, the CPT manual undergoes regular annual updates to reflect the latest in medical practice and innovations.

 280.

 281.

 282.
Providers are tasked with the responsibility of understanding, documenting, and reporting CPT codes accurately for the services they deliver, adhering to the extensive coding requirements outlined in the CPT manual.

 283.

Instead of performing the time-consuming process of figuring out why some claims are denied and how to appeal them, they leave a substantial amount of money untouched.
