  595. <p style="text-transform:capitalize">Nomkhosi Mkhwanazi, 35, is sitting in her office at Hlabisa Hospital, which is named after a small rural settlement in KwaZulu-Natal about a three-hour drive north of Durban.</p>

  599. <p style="text-transform:capitalize">She&#8217;s the only dietitian at the hospital.</p>

  603. <p style="text-transform:capitalize">Mkhwanazi grew up in Hlabisa and has lived there for the majority of her life.</p>

  607. <p style="text-transform:capitalize">Since 1999, the Umthombo Youth Development Foundation has assisted around 500 health workers in studying health science. Students from poor, rural backgrounds, like Mkhwanazi, are funded and mentored during their studies.</p>

  611. <p style="text-transform:capitalize">In exchange, they must work at hospitals in (or near) the towns or villages where they grew up for the same number of years that the foundation supported them.</p>
  612.  
  613.  
  614.  
  615. <p style="text-transform:capitalize">Mkhwanazi was required by contract to work in the Hlabisa area for four years, but she has stayed much longer. She will have worked here for nine years this month.</p>

  619. <p style="text-transform:capitalize">Her desire to help people in her community is, in fact, what inspired her to become a dietitian in the first place.</p>

  623. <p style="text-transform:capitalize">&#8220;I&#8217;ve always wanted to come back here and work,&#8221; Mkhwanazi says. &#8220;I was a student nurse here at the hospital before becoming a dietitian.&#8221; I noticed that many patients did not gain weight as a result of their illnesses or financial difficulties.&#8221;</p>

  627. <p style="text-transform:capitalize">Dietitians advise such patients on what foods to eat based on scientific evidence. &#8220;If patients are too sick to eat, dietitians will prescribe nutritional supplements,&#8221; Mkhwanazi adds.</p>

  631. <p style="text-transform:capitalize">If Mkhwanazi&#8217;s patients lived in cities like Cape Town or Durban, they would have had access to a variety of dietitians at public hospitals.</p>

  639. <p style="text-transform:capitalize">&#8220;Until I started working here in 2013, there wasn&#8217;t a single permanent dietitian at the hospital,&#8221; Mkhwanazi says. &#8220;So I decided to study dietetics and then return to my community to serve.&#8221;</p>

  643. <p style="text-transform:capitalize">Doctors who were raised in rural areas are more likely to work there.</p>

  651. <p style="text-transform:capitalize">People who grew up in rural towns are far more likely than their urban counterparts to work in desperately needed rural clinics and hospitals.</p>

  655. <p style="text-transform:capitalize">A 2019 study of Wits University medical students found that five years after graduation, doctors raised in rural areas were nearly five times more likely to be working in rural hospitals than those raised in cities.</p>

  659. <p style="text-transform:capitalize">Other South African medical schools&#8217; research has yielded similar results.</p>

  663. <p style="text-transform:capitalize">So what about Umthombo&#8217;s own information?</p>

  667. <p style="text-transform:capitalize">According to a 2015 study, nearly 95% of the 185 health science students who graduated by 2013 either fulfilled their contracts and worked back the entire period they were supported for at a rural health centre, or were in the process of doing so. And, of those who had completed their obligations, 71% continued to work in these fields.</p>

  671. <p style="text-transform:capitalize">The graduates of the foundation bring skills that rural hospitals and clinics struggle to attract or retain, according to a 2021 study published in BMC Health Services Research.</p>

  675. <p style="text-transform:capitalize">According to a 2011 health department report, only 3% of the 1200 medical students who graduated each year ended up working in rural areas 10 to 20 years later. There is no new health department data, but a 2019 study of doctors who graduated from Wits between 2007 and 2011 discovered that only 7% worked in rural areas.</p>

  679. <p style="text-transform:capitalize">This contributes to a significant disparity in how easily people in remote areas can access healthcare compared to those in the country&#8217;s major cities.</p>

  683. <p style="text-transform:capitalize">But why do people who grew up in rural areas and now have the means to live anywhere in South Africa (or even emigrate) choose to work in small South African settlements, often in under-resourced facilities?</p>

  687. <p style="text-transform:capitalize">Anecdotally, Bhekisisa visited three hospitals in rural KwaZulu-Natal where Umthombo graduates currently work. We spoke with ten of them, all of whom grew up in these neighbourhoods.</p>

  711. <p style="text-transform:capitalize">Mthokozisi Gumede, a social worker at Bethesda Hospital in Ubombo, a small village about a four-hour drive from Durban, says it gives him great satisfaction to serve people he grew up with.</p>

  715. <p style="text-transform:capitalize">Thulani Ngwenya had a similar concern after qualifying as a doctor and starting work in rural KwaZulu-Natal in 2013. Serving people in uMkhanyakude, the district municipality where he grew up and now works, was always important to him.</p>

  719. <p style="text-transform:capitalize">But he wanted to get away from his hometown of Ingwavuma, a rural village near the province&#8217;s northwestern tip, where it borders Eswatini. Ngwenya is now the medical manager at Bethesda Hospital, which is about 80 kilometres from Ingwavuma.</p>

  723. <p style="text-transform:capitalize">&#8220;A mother or grandmother in my culture would not feel comfortable being examined by someone they saw growing up,&#8221; he explains.</p>

Teachers can retire at the age of 55 and, in most cases, must retire by the age of 60, though in some cases, a limited number are permitted to remain in the classroom until the age of 65. This year, at least 12 000 student teachers were funded.<br><br><br>Poppie Ntaka, the study&#8217;s author, discovered that while universities are increasing teacher training outputs, provinces are not hiring them. Instead, classroom teacher-learner ratios were rising as a result of provinces&#8217; inability to hire due to budget constraints.<br><br>&#8220;While teacher production has increased significantly, provinces have not increased the number of teachers hired, and this gap has grown over time. Only 14 524 teachers were hired by provinces in 2021, while 28 335 graduated from universities, implying that only half of graduating teachers were hired. This was 75% between 2015 and 2016 &#8220;Ntaka stated this.</p>

  819. <p style="text-transform:capitalize">A cream, two-story building with a bright rainbow sign towers up about a five-minute drive off the Borcherds Quarry Road off-ramp to Crossroads near Cape Town on the N2 highway, behind an amagwinya (vetkoek) shop and a tin shack.<br><br>It is heavily secured, with four security guards wearing orange reflective vests patrolling the perimeter. CCTV cameras and additional guards watch over the Emavundleni Prevention Research Centre, where Amanda Roberts, 23, is scrolling on her phone while waiting for a doctor in a consulting room.<br><br>It&#8217;s pouring outside and the streets are muddy, but that hasn&#8217;t stopped Roberts, a teaching assistant, from coming to get a CAB-LA jab every two months. The shot, which will be injected into her buttocks, will save her life because it eliminates the possibility of her contracting HIV through sex.<br><br>Roberts&#8217; mother died of HIV in 2016. Her mother&#8217;s surviving twin sister has also become infected with the virus. &#8220;I&#8217;m afraid of getting infected and don&#8217;t want to follow in their footsteps,&#8221; she says. &#8220;I was abandoned with my younger brother and now have to care for him on my own.&#8221;<br><br>Roberts is an outspoken woman with long, dark-red braids draping down her back and a gold nose ring.<br><br>&#8220;I&#8217;m not embarrassed to come to the clinic,&#8221; she says. &#8220;Everyone knows I&#8217;m going to get this shot.&#8221;<br><br>Roberts and her boyfriend split up for four months a few years ago.</p>

  827. <p style="text-transform:capitalize"><br><br>&#8220;During that time, he must have slept with someone else,&#8221; she explains, &#8220;because I contracted a sexually transmitted infection shortly after we got back together, and he was the only person I had sex with.&#8221;<br><br>&#8220;I thought to myself, what if he had slept with someone who was HIV positive?&#8221; she says as she checks a WhatsApp message.<br><br>&#8220;I was desperate to get this shot.&#8221;<br><br>The Desmond Tutu HIV Centre at the University of Cape Town runs the Emavundleni Prevention Research Centre, which is a world-class clinical trial site. The open-label phase of a study called HPTN 084 is taking place here. The researchers and trial participants both know which drug volunteers receive in an open-label study.<br><br>CAB-LA is given to Roberts and over 3200 other women from Sub-Saharan Africa in this study. Because scientists have already established that the medication works extremely well in preventing HIV infection, researchers are looking at how easy or difficult it is to roll out.<br><br>Once the drug has been approved by our medicines regulator and a price has been agreed upon, this information will assist the government in determining what resources it will require and what obstacles it can expect along the way when (and if) the injection is eventually made available to hundreds of thousands of people.<br><br>Roberts has been taking CAB-LA for six months, and this is her third injection.<br><br>&#8220;I was desperate to take this injection,&#8221; she says. &#8220;I knew I was going to need it.&#8221;<br><br>The less frequently you take the pill, the less effective it becomes.<br><br>CAB-LA is an abbreviation for long-acting (LA) cabotegravir — cabotegravir is an antiretroviral drug (ARV) that some HIV-infected people use in combination with other ARVs to prevent the virus in their bodies from replicating itself. When such medication is used to prevent HIV infection — that is, before HIV exposure — it is referred to as pre-exposure prophylaxis, or PrEP.<br><br>&#8220;It works a little bit like a contraceptive injection in that it prevents you from getting pregnant,&#8221; says Linda-Gail Bekker, director of the Desmond Tutu HIV Centre and one of the lead investigators on the HPTN 084 trial in South Africa. &#8220;The injected drug [cabotegravir] enters the muscle and remains there for two months before slowly dissolving into the bloodstream.&#8221; Then it goes to work, preventing HIV from entering your cells.&#8221;<br><br>For five years prior to taking CAB-LA, Roberts took a daily HIV prevention pill containing two ARVs, tenofovir and emtricitabine.</p>
