<p><span style="font-weight: 400;">基本上肌少症本身不會直接引起疼痛,但它卻可能間接導致引發疼痛的狀況。</span></p>
<p><span style="font-weight: 400;">肌少症是指肌肉量減少</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">肌力減退及體能下降的狀況</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">這種情況不同於正常老化過程</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">是具有疾病的意涵</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">對健康有負面影響。</span></p>
<h2><b>但肌少症是如何 </b><b><i>間接 </i></b><b>導致疼痛:</b></h2>
<ol>

<h4><strong><strong>肌肉無力導致關節壓力增加</strong></strong></h4>
<p><span style="font-weight: 400;">我們的關節周圍包覆著許多肌肉</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">正常受壓時</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">肌肉出力可以幫忙減少關節的壓力</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">但是當你的肌力無力時</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">肌肉就無法支撐關節</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">這樣關節的受力將提升好幾倍。就像大腿前側(股四頭肌)肌肉力量每減少1 Nm/kg,膝關節在走路時的壓力會增加約10%~15%</span><span style="font-weight: 400;">,</span><span style="font-weight: 400;">同時膝關節軟骨與半月板受到的壓力大幅上升</span><b>,</b><span style="font-weight: 400;">長期可能加速磨損與退化。</span></p>

<h4><span style="font-weight: 400;"><span style="font-weight: 400;"><strong>姿勢不良</strong></span></span></h4>
<p>我們身體的姿勢全靠各肌肉間的力量協調來維持,某些姿勢不良就會造成特定的疼痛,就像核心無力者容易骨盆前傾,而骨盆前傾又會造成下背痛;又像臀肌無力者比較會出現膝蓋內夾的情形,最後造成膝蓋疼痛。長期下來就會引發發炎及疼痛。</p>

<h4><span style="font-weight: 400;"><span style="font-weight: 400;"><strong>跌倒與受傷風險增加</strong></span></span></h4>
<p>因為下肢肌肉力量不足,無法在失去平衡的一瞬間利用大腿跨步來維持平衡將增加跌倒風險,而跌倒可能導玫骨折、軟組織受傷或是慢性疼痛症候群。</p>

<h2><strong>肌肉</strong></h2>
<p><span style="font-weight: 400;">對於大多數人(非運動員)來說, 不活動會導致肌肉力量以每天 1% 到 3% 的速度下降, 大約兩周半到三周後會出現明顯的力量損失。如果您沒有生病並且能夠進行一些運動和輕度運動,您可以將這個衰退延長到四到五周,而不會明顯失去力量。如果您生病或完全無法動彈(例如臥床休息),肌肉力量可以在短短三周內下降 50%。</span></p>
<h2>心血管系統</h2>
<p><span style="font-weight: 400;">我們失去心肺耐力的速度比失去肌肉力量的速度還要快。當您定期鍛煉時,心臟的每一次跳動都可以擠壓出更多的血液。當您身體狀況不佳時,您的心臟會逐漸失去這個的能力(在24小時內最多可失去5%),並且您的休息心率會在3到4周內增加4到15次,然後才會達到平臺期。另一項體能研究顯示,規律訓練兩個月獲得的 VO2 max(身體的最大氧氣攝入量-通常代表體能的指標)增加會在一個月的不活動後會完全消失。</span></p>
<h2><strong>體重</strong></h2>
<p><span style="font-weight: 400;">當您從日常鍛煉中長時間休息時,您的新陳代謝會開始減慢,您每天燃燒的卡路里數量會發生變化。這意味著您需要調整卡路里攝入量以避免體重增加, 否則任何超出您身體可以燃燒的卡路里的額外卡路里都將被儲存為脂肪</span></p>
<h2><strong>我可以如何做?</strong></h2>
<p><span style="font-weight: 400;">好消息是:這些不活動的對於肌肉、心血管系統和體重的影響都可以通過再運動來逆轉。壞消息是:你不會在幾天內恢復體形。加強肌肉、增加最大 VO2 和減輕體重可能需要兩~三倍的時間來恢復。</span></p>
<p><span style="font-weight: 400;">因此最好的方法是規律的運動, 不要讓他有衰退的情形, 以下是一些可行的小方法</span></p>
<ol>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">避免全有全無: 如果真的擠不出30分鐘做運動, 至少拆成2個10分鐘來動一動, 做點什麼都好而不是什麼都不做</span></li>
<li style="font-weight: 400;" aria-level="1"><span style="font-weight: 400;">自重鍛鍊: 並不需要進健身房用很重的重量才是阻力訓練, </span><span style="font-weight: 400;">而是用自己的體重作為力量訓練的阻力即可。俯卧撐、深蹲、弓步等都是您幾乎可以在任何地方進行體重鍛煉的例子。</span></li>
</ol>
<p><span style="font-weight: 400;">接受你現在的能力</span><b>: </b><span style="font-weight: 400;">過快開始鍛煉可能會導致受傷, 並導致您永遠放棄鍛煉。以現在可以負荷的能力開始嘗試, 不要老想著我以前可以怎樣又怎樣。</span></p>

<p>記者趙于婷/台北報導</p><p>類風濕性關節炎是常見的自體免疫疾病,全台約有10萬名患者,嚴重患者會面臨關節疼痛變形。一名65歲的早餐店老闆娘徐小姐手指關節變形10多年,忍到嚴重疼痛才就醫,經核磁共振及X光檢查發現有骨性侵犯,發炎指數過高。 除了規律服用西藥之外,搭配中醫治療,包括洗劑、正骨手法等,才大幅緩解關節疼痛腫脹。</p><p>臺北榮總傳統醫學部龔彥穎主任和盧禾潾醫師團隊研究發現,類風濕性關節炎病人除規律服用抗風濕性西藥、生物製劑及消炎藥物外,搭配針灸、徒手治療及外用中藥洗劑等中醫整合治療,更可幫助改善發炎指數、關節疼痛腫脹。該研究結果也獲2021年《Journal of the Chinese Medical Association》 雜誌刊登。</p><p>盧禾潾指出,類風濕性關節炎是一種慢性漸進式破壞關節的自體免疫疾病,常侵犯手部小關節,病人深受關節疼痛、變形及影響日常生活所苦,臨床上治療目標是緩解疼痛,抑制發炎及減緩關節的破壞,一般來說,西醫主要使用抗類風濕性西藥、生物製劑及消炎藥物治療,由於會消化道不適等副作用及費用較高,臨床上有許多病人轉求助於中醫的幫助。</p><p>盧禾潾說,中醫治療關節炎也會使用傳統傷科「整復徒手治療」,可經由關節鬆動術及正骨手法,幫助紊亂關節恢復對位,對於類風溼性關節炎的變形、疼痛具有舒緩效果。</p><p>除了針灸、傷科整復外,北榮中醫傷科團隊也研發「傷科洗劑」,由「防風、荊芥、川芎、甘草、當歸、桂枝、黃柏、透骨草、蒼朮、丹皮、細辛和絡石藤製成,具有疏散風濕、活血通絡、消炎止痛」等功效,特別適合關節炎患者使用。</p><p>盧禾潾提醒,該洗劑藥方須由醫師診斷開立,各藥材有不同的功效,會全部打成粉放在藥包裡,早、晚使用一次,使用時可用38度~40度溫熱水泡,然後將手放到水裡,可用毛巾幫助包覆,泡10分鐘左右,達到熱療效果,患者自己也可以針對特別關節做關節活動。</p><p><div class="ast-oembed-container" style="height: 100%;"><iframe loading="lazy" title="類風溼性關節炎所苦 中西醫合療成效佳" width="1200" height="675" src="https://www.youtube.com/embed/cKkVNf0jaro?feature=oembed" frameborder="0" allow="accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share" referrerpolicy="strict-origin-when-cross-origin" allowfullscreen></iframe></div></p><p>原文網址: 自體免疫疾病作祟!早餐店闆娘「中指腫脹」嚴重變形忍10多年 | ETtoday健康雲 | ETtoday新聞雲 <a href="https://health.ettoday.net/news/2444837#ixzz8seMesHPO">https://health.ettoday.net/news/2444837#ixzz8seMesHPO</a></p> </div>

徐小姐今(2/21)出席記者會現身說法,提起自己病況一度哽咽落淚。她表示,罹患類風濕性關節炎好幾年,被這病深深折磨。因為手要出力卻沒法使力,很多事情都沒法做,需要別人幫忙,造成困擾,再加上她做早餐非常忙,所以覺得很辛苦。接受中醫治療前,她的左手已無法拿東西、手腕腫到沒法動,再加上長期吃西藥消炎止痛劑,腸胃很不舒服、睡眠也被影響。用中醫調理7~8年,她的左手腕已恢復正常,西藥也只有打生物製劑、一般止痛藥很少吃。徐小姐說,每天早晨起床,雖然手指關節還是很僵硬,但是泡中藥材藥洗,早晚各泡10分鐘,疼痛就減少、舒服很多。 </div>
