Packages that go missing after confirmed delivery by third-parties are often stolen, and Medicare will pay for replacement items when the originals are lost, stolen, or irreparably damaged.</p><h2>Getting Paid to Replace Stolen Items</h2><p>Medicare requires documentation confirming the theft. The best support is an official police report, but a formal written statement from the customer alleging theft and detailing relevant facts may suffice.</p><p>Once the supplier has documentation of the theft, they can provide identical – or nearly identical – items to the Medicare customer. To obtain reimbursement for the replacement items, suppliers should:</p><ol><li>Append the RA modifier to the claim for replacement items, and</li><li>Include the narrative “Replacement due to theft.”</li></ol><h2>Repeated Thefts</h2><p>The Medicare replacement policy is simple, but contractors will question repeated thefts, especially within a relatively short period of time … say 18-24 months. To avoid customer abuse and Medicare’s suspicion, suppliers should have a policy for dealing with customers claiming frequent losses. A good policy starts with educating customers on the importance of retrieving parcels promptly, and to the extent possible, creating a secure delivery drop for mailed packages. If shipments continue to go missing, suppliers should consider discontinuing delivery to the risky location.</p><p>We hope thieves don’t take advantage of Medicare beneficiaries, but when they do, we hope this will help suppliers ensure continued patient access without unfair financial consequences.</p> </div>

Many devices put into use during the PHE are now reaching the end of their reasonable useful lifetime (RUL), making it essential to assess replacement strategies and long-term readiness. <a href="https://miravistallc.com/?s=oxygen">O</a></span><a href="https://miravistallc.com/?s=oxygen"><span style="font-weight: 400;">xygen</span></a><span style="font-weight: 400;"> suppliers are almost certainly wondering,</span></p><p><span style="font-weight: 400;"><i>“Does Medicare require a new qualifying blood gas study and chart notes to provide replacement equipment?”</i></span></p><p><span style="font-weight: 400;">In short, Medicare does not require requalification under the normal coverage rules if the supplier:</span></p><ul><li><span style="font-weight: 400;">Set up the initial oxygen equipment between March 1, 2020, and May 11, 2023,</span></li><li><span style="font-weight: 400;">Relied on PHE clinical waivers to qualify the patient, and</span></li><li><span style="font-weight: 400;">Included the CR modifier and COVID-19 narrative on billed claims.</span></li></ul><p><span style="font-weight: 400;">And &#8211; bonus &#8211; unlike many other product categories, Medicare will replace oxygen equipment at the end of its five-year RUL even if the original device is still functioning.</span></p><p><span style="font-weight: 400;">To qualify oxygen patients for replacement equipment, suppliers must:</span></p><ul><li><span style="font-weight: 400;">Document continued use,</span></li><li><span style="font-weight: 400;">Obtain a new order from the patient’s physician to establish current medical need,</span></li><li><span style="font-weight: 400;">Add the RA modifier and applicable narrative to the claim for the first month’s replacement, and</span></li><li><span style="font-weight: 400;">Continue to use the CR modifier and COVID-19 narrative on claims for replacement equipment.</span></li></ul><p><span style="font-weight: 400;">Ultimately, the clinical waivers that permitted patients to qualify for oxygen during the PHE carry over to new equipment if the device remained in continuous use from original setup. In those cases, <a href="https://nikohealth.com/medical-resupply/">suppliers</a> do not need to obtain chart notes and test results that support coverage under the normal guidelines.</span></p> </div>

  250. <p>DME suppliers are shocked to find out Medicare has deactivated their billing number. The most common reason for a deactivated <a href="https://miravistallc.com/?s=PTAN">PTAN</a> is – far and away – the supplier’s failure to respond to an inquiry from one of the National Provider Enrollment contractors (NPEs).</p><p>Fortunately, that is something suppliers can proactively protect themselves against.</p><p><b>Never Ignore NPE Development Requests</b></p><p>That much is obvious.</p><p>It is a bad idea to outright ignore any request from any Medicare contractor. Failing to respond is effectively withholding information from Medicare, and that violates Supplier Standard 21:</p><p>“A supplier must agree to furnish CMS any information required by the Medicare statute and regulations.”</p><p>But in our experience, suppliers don’t often disregard requests. Instead, they respond and <i>assume</i> the requesting agency has what it needs without any confirmation or follow up. Medicare agencies do not distinguish between no response and an incomplete one. If the agency doesn’t get what it needs, it initiates deactivation.</p><p><a href="https://nikohealth.com/medical-resupply/">Medical Resuppliers</a> should call the NPEs to make sure they understand the request before responding, and they should follow up afterwards to make sure the matter is <i>resolved </i>before the 15-day development window closes.</p><p><b>Do Not Overlook NPE Requests Sent via Email</b></p><p>“But I never received it!” doesn’t cut it.</p><p>NPEs typically send development requests via email to the point of contact listed in PECOS. Suppliers should regularly verify the PECOS contact name and email address is current. Points of contact, for their part, should check their spam folders weekly to make sure they don’t miss time sensitive messages, especially when any PECOS application is pending.</p><p><b>Medicare Requires Revalidation Every Three Years</b></p><p>Suppliers should be on high alert after submitting any application to modify PECOS, and should remain so until Medicare completes the change. Regular revalidation, however, can sneak up on distracted suppliers. To avoid missing information requests related to revalidation, suppliers should check the CMS website at least twice each year to avoid missing Medicare-initiated development requests.</p><p>To check for an upcoming revalidation due date:</p><ol><li>Visit the Medicare Revalidation List at<a href="https://data.cms.gov/tools/medicare-revalidation-list"> https://data.cms.gov/tools/medicare-revalidation-list</a>.</li><li>Query the supplier using the NPI or company name.</li><li>Click “Find a Provider.”</li></ol><p>Revalidation is due on the date listed by “Due Date” if the date is in the future. NPE contractors accept revalidation applications up to seven months before the posted due date.</p><p>“TBD” means there is no pending requirement. Do not initiate revalidation when there is no date listed.</p><p>Ultimately, a comprehensive PTAN deactivation prevention strategy requires suppliers to:</p><ol><li>Check the Medicare Revalidation List semi-annually.</li><li>Maintain high alert when any PECOS activity is pending.</li><li>Ensure PECOS records list current contact information for individuals responsible for responding to Medicare requests… and those individuals regularly check their spam folders for communications and development inquiries.</li><li>Respond to all requests.</li><li>Follow up regularly with requesting agencies to ensure favorable resolution.</li></ol> </div>
