Only 10% of pubs intervene to stop children gambling on their premises, the Gambling Commission has found.
The watchdog said it had found a “concerning” number of pubs where youngsters used gaming machines.
Pub staff are expected to prevent under-18s playing on the machines and there should be clear signage indicating the age restriction.
The British Beer and Pub Association said it took the findings “very seriously”.
The Gambling Commission said it had been working with local police and licensing authorities to check pubs were acting within the law.
It said the 89% rate of failure – found across a sample of pubs – was much worse than the 15% to 30% failure rate seen in other industries selling age-restricted products, such as alcohol or tobacco.
Helen Rhodes, programme director at the Gambling Commission, said: “We are extremely concerned that pubs across England are failing to stop children playing gaming machines designed for adults.
“We urgently call on the pub sector to take action immediately to enforce the laws in place to protect children and young people.”
Brigid Simmonds, chief executive of the British Beer and Pub Association (BBPA), said that pubs should offer a “safe and friendly environment for families”.
“We have ensured that all of our members are aware of both the BBPA’s and Gambling Commission’s codes of practice and we are already taking steps to develop a social charter for responsible gambling, for use by licensees and pub companies.
“However given the importance of this issue we are seeking urgent meetings with the Gambling Commission and local authorities to ensure appropriate action is taken.”
On Tuesday the government promised to bring forward a cut to the maximum bet on fixed-odds betting terminals (FOBTs) amid pressure from MPs.
It had originally planned to reduce the stake from £100 to £2 in October next year, but will now do so from April.
FOBTs have been criticised for encouraging excessive gambling and exposing people to the risk of addiction, and the social, mental and financial problems associated with gambling.

A Channel Islands authority has been accused of taking advantage of people’s gambling problems to boost its revenue.
Deputy Emilie Yerby said the States of Guernsey profited from the sale of Channel Islands Lottery scratch cards, with some shops asking customers if they would like to buy one at the till.
Earlier this month Ms Yerby’s motion to debate gambling was rejected.
According to the latest figures, the States of Guernsey made £9.6m from the sale of scratch cards in 2017.
In 2007 the authority generated £822,000, an increase of more than 1,000% in 10 years.
‘Element of social responsibility’
Thousands of pounds raised from the Channel Islands Lottery goes towards the running costs of Beau Sejour Leisure Centre in St Peter Port, Guernsey.
Ms Yerby said she thought it was a mistake for the States to depend on lottery cash to pay for one of its core services.
“There’s generally a reluctance in the States to debate anything that’s going to involve us having to raise money from different sources, or having to raise more money, which inevitably we would have to do to fill the gaps left by the lottery revenue,” she said.
“But I do also think there’s a real recognition that we have an element of social responsibility when it comes to gambling and we shouldn’t be taking advantage of people’s difficulties to prop up our own revenues.”
Last year Beau Sejour Leisure Centre received £493,000 from the Channel Islands Lottery.
More than £7m was paid out in prize money with another £2m spent on costs and the balance held for future projects.
In January, the States agreed to make a £15,000 payment to the Committee for Home Affairs every year from the lottery to support people experiencing gambling problems.

George Proud from County Durham, a keen online gamer, spent nearly £700 in a month opening loot boxes as he progressed through a game.
The boxes – which offer players new weapons or character skins – can be opened by playing the game for a long time or by paying money.
George says they’re addictive: “Over time you will gamble money away trying to get the item you want,” he told Radio 4’s You & Yours.
Popular games such as Overwatch and Call of Duty: Black Ops 4 are just some of the dozens of games that have loot boxes in them.
The issue of whether paying to open loot boxes constitutes gambling behaviour is an issue currently being looked at by the government and the industry regulator, the Gambling Commission.
Last week the Digital, Culture, Media and Sport Committee heard from researchers who said they needed games companies to hand over player data so they could conduct thorough studies into the issue.
A survey of more than 2,000 adults conducted by market research company Populus adults found that while only one third had heard of loot boxes, 74% of people who were aware of them believed loot boxes blurred the lines between gaming and gambling.
Almost three-quarters (72%) agreed they teach children how to gamble, and 68% agreed they should be classed as gambling.
Conservative Peer Lord Chadlington, who commissioned the survey, said loot boxes should be classed as gambling.
“Notice the words George Proud uses. He said, ‘I’ve gambled away this money’, and that’s what happens.
“Because you don’t know in many cases when you go into the loot box what you’re going to get, and if you use money this creates the risk feeling.”
He added: “I’m only concerned about one thing, I’m not fighting the gambling companies or gaming companies or anyone else. We just have to be very careful we do not ignore evidence which suggests that there will be a gambling epidemic among young people in the years ahead.”
The Gambling Commission says current regulation does not cover loot boxes because the prize in a loot box is not worth real money.
However, Radio 4’s You & Yours has seen loot boxes and the contents of the boxes being bought and sold in online marketplaces. Some items that are really desirable can fetch hundreds of pounds.
The regulator said it’s looking into this problem, as where in-game items from loot boxes can be readily exchanged for cash, the loot boxes could fall within the definition of gambling.
Players can end up spending a lot of money to get the item they want. There’s also the added pressure from other gamers to get a unique or limited edition reward in the loot box.
Australia, the Netherlands and Belgium have now regulated loot boxes under gambling laws. Those countries believe that loot boxes replicate gambling behaviour, because the gamer is repeatedly spending money to try and hit a jackpot.
However, in the UK current legislation doesn’t allow loot boxes to be regulated under gambling law as no money is “cashed out” as a prize.
The Gambling Commission said: “We are concerned with the growth in examples where the line between video gaming and gambling is becoming increasingly blurred and where we are clear that a product has passed the line into gambling and is posing a risk to players, we will take action.
“In 2017 we successfully brought the first criminal prosecution in this area in relation to Futgalaxy who we prosecuted for providing skins gambling to the underage.”

Health groups, charities, regulators and businesses are gathering to tackle problem gambling.
The Gambling Commission says it will reduce gambling-related damage “much faster” under its three-year strategy.
The national strategy focuses on prevention, education and treatment and support for problem gamblers.
Ahead of the announcement, the two British betting companies said they would change their advertising practices as part of an attempt to address the issue.
GVC, which owns Ladbrokes, Coral and Gala, and William Hill say they are stopping football shirt sponsorship. GVC is also stopping perimeter adverts.
And it has called for an end to all UK sports-betting broadcast advertising, at any time of the day, on live and repeated sporting events.
However, GVC’s proposed ban on TV advertising did not include betting adverts during horseracing, which is seen as critical to the betting industry’s profitability.
It follows increasing pressure from regulators and government to make betting companies take action to curb problem gambling.
Bookmakers Paddy Power and Betfred both pulled new high stakes roulette-style games after a warning from the Gambling Commission.
Gambling profits have been squeezed after the government forced betting companies to lower the maximum stake that can be played on their fixed-odds betting terminals (FOBTs) from £100 to £2.
Last year William Hill was hit with a £6.2m penalty package for breaching anti-money-laundering and social responsibility regulations.
‘Serious health issue’
The Gambling Commission estimates that about 430,000 people are experiencing problems with gambling.
The NHS believes there is increasing evidence of a link between problem gambling and stress, depression and other mental health issues.
Marc Etches, head of the GambleAware charity, which contributed to the Gambling Commission’s National Strategy to Reduce Gambling Harms, said: “Gambling is a serious public health issue in Britain with two million adults suffering some level of gambling-related harm, and for a few, gambling addiction ends in suicide.
“Commitments by some in the gambling industry to reduce advertising and to increase funding for research and the prevention and treatment gambling addiction are welcome but there is much more that can and should be done across the industry as a whole.”
The Minister for Sport and Civil Society, Mims Davies, said: “Protecting people from harm should be at the heart of every gambling business. Addiction can ruin lives and it is vital that those who need help are given the right treatment at the right time.”
Much of the new strategy concentrates on gathering information and research on the effects of gambling. The Commission is considering setting up a National Research Centre and has already started building a national database.
William Moyes, chairman of the Gambling Commission, said: “We all need to better understand the harms that can be caused by gambling, moving away from simply counting problem gamblers and instead build a greater understanding of the harms experienced.”
Public Health England will carry out the first review of evidence on how gambling affects public health, which will be published in spring next year.
Rosanna O’Connor, director of alcohol, drugs, tobacco and justice at Public Health England, said: “There is an urgent need to develop a better understanding of these harms and how best to respond to them.”
Meanwhile the betting industry has taken steps to curb problem gambling. From August, it will stop TV advertising during most live sport.
William Hill launched its “Nobody Harmed” programme last year to help prevent problem gambling, and appointed a group director of strategy and sustainability, Lyndsay Wright, to lead it.
Philip Bowcock, chief executive of William Hill, wrote in the Daily Telegraph that switching off customers from its gambling systems for safer gambling reasons had cost it an estimated £17m.
He wrote that he believed the company should continue to tackle problem gambling: “If that means taking some short-term commercial hits, so be it. Our future relies on customers who enjoy gambling and stay gambling with us for the long term – and that is dependent on them being safe.”

David Chow Kam Fai, the company’s co-chairman and chief executive officer, said on Wednesday. He also promised to “speed up” the development of the Laos casino resort project next to the Saban Legend Resort Hotel and Entertainment Complex, which is currently operating in Laos.
At the company’s annual general meeting, Mr Chow said Macau Legends was buying land to develop a “integrated resort” project in Cambodia. He did not elaborate on the casinos. He added that it was an “ongoing process” that included help from an “international law firm.”
“The land purchase is almost complete. Regarding important issues such as project size and other related questions, I believe we will be able to disclose them in two to three months,” Mr. Chow said, referring to the Siem Reap plan. He did not give further details about the facilities.
Siem Reap is a popular tourist destination close to the ancient temple complex in Angkor Wat, a UNESCO World Heritage Site.

The funds will be used for the second phase of the Paradise City Casino Resort (pictured), which will be opened in phases starting in September this year, Paradise said in a filing with the Korea Exchange on Monday.
The casino operator said it would pay 55 billion won for about 5.5 million new shares of Paradise Segasami. Segasami Holdings will acquire nearly 4.5 million shares, taking into account a total of 45 billion won.

The South Korean company said the second phase and first phase of Paradise City Casino Resort will focus on out-of-game amenities, including more hotel rooms, retail stores, clubs and spas.

Paradise City opened in 2017.
