  49. <content:encoded><![CDATA[<p> Early December, 2010 I was contacted by Mr. Stephen Will of The Hendra Agency in New York, New York.  Mr. Will had read one of my articles online concerning Lean Healthcare.  The Hendra Agency represented Jim Champy, co-author of Reengineering Health Care, A Manifesto for Radically Rethinking Health Care Delivery.  Mr. Will proposed that I chat with Mr. Champy, since we seemed to have many of the same interests.  I agreed and on January 5 Mr. Champy and I discussed our mutual concerns about health care.  Following are some excerpts from that discussion as well as a review of some of the material in his book.December 21, 2010 Laura Landro, healthcare columnist for the Wall Street Journal, recommended Reengineering Health Care written by Jim Champy and Harry Greenspun, M.D., as one of two must-reads for healthcare executives.  The other was Transforming Health Care:  Virginia Mason Medical Center&#8217;s Pursuit of the Perfect Patient Experience.  Both books advocated focusing on the quality and safety of patient care.  Both books recommended major changes in the way patient care is delivered.In my discussion with Jim Champy I asked if his approach to changing healthcare was similar to the Toyota Production System (Lean Healthcare) and if so, what was the difference (I found his recommended approaches quite similar to the TPS when I read the book).  He stated that the approaches he illustrated in the book were quite similar to the TPS but that the difference was that he advocated such dramatic changes in a shorter time frame.  The TPS can take 4 or 5 years to significantly change the culture and processes at a healthcare site.  According to Mr. Champy, such a gradual approach is too long given the inefficiencies, costs and safety and quality issues plaguing healthcare today.  He supports reengineering healthcare in a much shorter time frame.Mr. Champy in our discussion and in his book advocated that clinicians lead the change at healthcare sites.  In order to meet the time frame he advocates change must be directed by clinicians and senior executives.  One of the early chapters of the book illustrates this approach by detailing the changes Zeev Neuwirth, M.D., brought about in his career.  Dr. Neuwirth early in his career instituted significant changes in patient care in the Bronx Veterans Administration hospital.  He spent his time there improving the patient experience by focusing on communication and team building.Another section of the book focuses on the lifetime work of Debra Geihsler, who was at different times VP of Mercy Health System in Wisconsin, president of the Advocate Medical Group in Illinois, and CEO of Atrius Health in Boston.  Ms. Geihsler focused in much of her work on building the capacities of physician groups.  At Mercy she helped build a unified system that provided services on many levels, just short of in-patient hospitalization.  The seamless integration of services provided much better communication among the multiple providers in the system, improving both the quality and safety of care delivered while significantly improving the bottom line.Jim Champy in his comments to me said he wrote this book with an eye to readability and easy application.  There are many books that give significant details, charts and graphs about improving the delivery of healthcare.  He wanted to avoid being too technical, making the book an easier read for executives and laymen.  Thus, he focused on real stories about individuals who have made significant changes in the way that care is delivered.  He focuses on three main elements of change in the book-technology, processes and people.  Technology provides significant opportunities for improved communication and viewing change at the population level, to see the progress of change.  Significant change cannot occur without viewing the delivery of care as a process.  Making these elements work requires commitment by individuals who have the means to carry out the change.I asked Mr. Champy how he approached overcoming barriers to change.  Physicians and other clinicians who have been in the system for some time are often reluctant to endorse changes and, thus, can significantly impede progress.  He stated that he would take such individuals to &#8220;come and see&#8221; the positive changes that have already occurred.  In the book, he says it is often easier to start changes with clinicians who have not been in the system too long.  They are more eager to welcome changes in the environment.  Starting with the younger clinicians, those who are directing the changes should after achieving significant gains, take those who are resistant and demonstrate how the new ways of delivering care have improved quality and safety and that those involved are happy with the achievements and their efforts in making the changes.  The resistant clinicians are usually willing to go along with the changes after seeing such positive outcomes.  After all, says Mr. Champy, most physicians and clinicians really want to deliver very effective care to their patients but do not understand how to make the changes necessary to deliver ever better care.I asked Mr Champy what one additional chapter he would like to add to the book if he could.  He said that he would like to add some additional focus on the continuum of care, such as advocated by Debra Geihsler.  The medical care that he receives is coordinated by his primary care physician.  He feels much better that the care he receives is not fragmented but is overseen by his personal physician.  He believes that the best care is based upon this patient-centered medical home model.I highly recommend this book for organizations that want to significantly improve the care that they deliver and want to understand quickly good ways to effect the changes necessary.  The book is a quick read and effectively describes for those who will be directing changes at their site what is necessary to accomplish their goals.  The book does not say the path is smooth and easy but that change can be effected through individuals who are dedicated and knowledgeable. </p>

Elder Financial Abuse. These are cases where financial predators use undue influence, manipulation and coercion to take property and money from elders who have diminished mental capacity. The perpetrators can by anybody, but are usually family members, caregivers or friends who have the most access to the elder and are least likely to be challenged by outsiders.- 2. Nursing Home Abuse. Abuse or neglect of a nursing home patient generally results from inadequate staffing where there are too many patients and not enough nursing assistants. These written instructions assist the elderly client both during their lifetime and afterwards. A proper estate plan can also avoid the need to establish a conservatorship if the elder becomes unable to make sound decisions. These documents grant all of the legal authority needed to carry on the elder&#8217;s affairs &#8211; without having to seek court involvement.- 5. Medi-Cal Planning. This term applies to California residents and involves the process of obtaining Medi-Cal benefits to pay the sky-rocketing costs of long term care in a skilled nursing facility. Planning involves three stages: (1) Thorough analysis of the client&#8217;s financial status to develop a written strategy to obtain eligibility, reduce any share of cost, and avoid recovery claims; (2) Execution of the written strategy; and (3) Completion of the application process. Medi-Cal law is complicated and requires a lawyer experienced in the nuances of the various rules and regulations.There are many other sub-fields that fall under the elder law umbrella. Each has its own peculiar application and importance as a piece of the elder law puzzle. I&#8217;ve even taken a couple of their comfortable wide fit shoes and made them into sparkling triumphs courtesy of Claire&#8217;s Accessories and some Superdrug nail varnish. I know, these are not names that generally inspire, but when the resulting sandals are mistaken for the latest season sandals from Dune (and are actually comfortable enough to walk in) I knew I was on to a winner.Fast forward to a girlie weekend in Barcelona this March and a visit to a local spa and suddenly my mind was working overtime again.As much as I love the idea of a long weekend at a luxury spa hotel, if I&#8217;m honest the prices have always put me off, and my recent experiences at day spas in Morocco and Barcelona have convinced me that you don&#8217;t need to pay through the nose for an exceptional spa experience. In fact the best massages I have had to date have been the cheapest ones:- Around 32 pounds for a hammam, with a gommage &#8220;scrubbing&#8221;, followed by a rhassoul, and then the most relaxing massage I&#8217;ve ever had. I can&#8217;t tell you how long I was in there &#8211; only that the sun had gone down, I had lost all track of time, and I was on a completely different planet.- Even in Barcelona, with the euro at an all-time high, the top rated spa cost us around 54 pounds for the hammam, (with extra hot steam room and plunge pool), followed by a gommage &#8220;scrubbing&#8221; and another extremely relaxing massage. Three hours of luxury for 54 pounds compared to UK prices is a total steal.The fact is that in many cultures the local spa is an everyday (or at least every month) treat, which, combined with good exchange rates (when they are available), makes them a great bargain on holiday. So why aren&#8217;t we taking advantage of them? Well personally I think it&#8217;s because, whereas guide books are stuffed with restaurant, sight and hotel recommendations, very rarely do they spend any time on spas. The spa in Morocco was recommended by friends, and the spa in Barcelona was found with some pre-emptive internet research. Later in the year I&#8217;m off to visit China, which has some world class hotel spas, but also boasts a proud massage tradition, which leads me to hope that as well as the day spas I&#8217;ve been recommended (where prices are about a quarter of UK rates) I&#8217;ll also be able to find other more authentic bargains.Which leads me to the hotel part of the spa and hotel break. One of the reasons that staying at a luxury spa hotel can cost so much is that you are paying for the option to use it 24/7. If, like me, you see your spa time as part of a whole trip rather than a destination in itself, you&#8217;ll probably find that one day spending 3-4 hours in a spa is enough (taking into account the detox effects) at least for a couple of days. So just by choosing to stay in a separate hotel you will probably save a hefty chunk of your holiday budget, and you&#8217;ll also avoid the temptation to stay locked up in your hotel for your entire stay and miss out on the local treats.By then choosing an affordable hotel, reasonably close to your spa, which features the required level of pampering (slippers, robe, fluffy white towels), you can relax back in your room safe in the knowledge that you&#8217;ll be going home with a designer glow and a budget bill.Of course this is not to say that there aren&#8217;t spa hotels offering affordable deals, but by being flexible and savvy you can make sure that your relaxation time isn&#8217;t spent feeling ripped off. And perhaps more importantly, by being more creative in choosing your hotel, you have a much better choice of spa. You are not limited to an in-house spa experience, very important considering the number of excellent day spas available, even in the UK. Take for example Spa London in Bethnal Green, voted 2010 Professional Beauty Awards &#8220;Day Spa of the Year&#8221;, where 3 hours of thermal experience will set you back 21.50 pounds at the top of the scale for non-members, down to 6.75 pounds for concessionary local members (including towel, robe and flip flop use) and which is truly a luxury experience compared to the slightly run down Porchester Spa. And then there&#8217;s Thermae Bath Spa, the only place in the UK where you can bathe in natural healing spa waters, voted 2010 &#8220;Best Spa in the World&#8221; by readers of the Daily Telegraph&#8217;s Ultra Travel Magazine for the second time running, where a 2 hour session in the New Royal Bath (recommended) will set you back 24 pounds (plus 9 pounds hire cost for towel, robe and slippers if you need them &#8211; and you get to keep the slippers!) Unlike many spas the Thermae Bath Spa also offers reductions for locals, disabled guests and their carers, so a local disabled guest (and carer) could pay as little as 9.50 pounds each for a 2 hour session in the New Royal Bath. (You can also buy a glass of spa water at The Pump Room for only 50p!)Of course London and Bath are not known for their cheap hotel rooms, and certainly in central Bath you might be pushed to find availability, let alone an affordable room rate. However, if you widen your search to the surrounding area, you&#8217;ll also find some wonderful B&#038;Bs which would put many hotels to shame, and where you can wake up to the sound of a horse clip clopping past, right in the heart of the countryside. Then it&#8217;s up to you whether you decide to explore some old fashioned villages, head back home refreshed and revived, or pop back to the Thermae Bath Spa for just a little more bargain relaxation. </p>
